*-Playoffs will consist of a best of 3 Championship Series.
Top 2 teams qualify in a 4 team division. Top 3 teams qualify in a 5 team division. Top 4 teams qualify in a 6+ team division. Semi Final games are 1 game winner advances.

AGREE: I acknowledge the fact that ice hockey can cause injury and has inherent danger, and that I am playing at my own risk! By submitting this form I also understand the terms and schedule of payments included herein and agree to meet fee deadlines. If I am late with any payment, I understand that I MAY BE RELEASED from my team and subject to late fees of $10 for each late payment. If I choose Credit Card as my method of payment and I choose the 2 or 3 payment plan, I agree to allow the IAHL to charge my credit card on the due dates in the amount listed. I also understand that if for any reason I CHOOSE TO QUIT in games 1-9, I will be charged in the amount of $30 per my teams games played and refunded any balance left over. If I CHOOSE TO QUIT in games 10-18, I understand that there are NO REFUNDS. I UNDERSTAND THAT I AM SUBJECT TO ALL RULES, REGULATIONS, AND REPURCUSSIONS that govern IAHL play, regardless of whether I have had the opportunity to read the rule materials offered.
